Job summary

Sweetwater Care in Columbia, SC seeks a dedicated Speech-Language Pathologist to assist patients with communication disorders and swallowing safety. You will develop and monitor treatment plans, perform assessments, and collaborate with a multi-disciplinary team to support patient goals.

Responsibilities include conducting evaluations, creating plans of care, and documenting progress in patient records in a SNF environment. Prior experience with Medicare/Medicaid reimbursement is preferred.

Qualifications

• Experience with standardized speech and language assessments.
• Knowledge of Medicare, Medicaid and other reimbursement systems.
• Experience providing speech-language pathology services in SNF settings.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, planning, goals, family education and documentation.
• Provide professional treatment for rehabilitation of speech, language, swallowing and cognitive disorders.
• Develop treatment plans and obtain physician approvals; monitor progress and adjust as needed.
• Collaborate with occupational, physical therapists and physicians on patient care.
• Document evaluations, progress notes and discharge summaries in a timely manner.
