Job summary

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to company guidelines and standards.
• Provides professional treatment for rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Serves as patient and rehabilitation advocate; participates in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans with short-term and long-term goals aligned with physician orders.
• Carry out patient care plans and progression of patients within the established plan.
• Demonstrates knowledge of Medicare, Medicaid and other reimbursement systems.
• Develops treatment plans and obtains approval for services from referring physician.
• Participates in patient care planning and utilization review meetings.
• Demonstrates knowledge to provide patient care in a SNF setting.
• Administers speech/language assessments, swallowing evaluations, and monitors progress.
• Performs tests and complex treatments individually or with assistance as needed.
• Collaborates with occupational/physical therapists, physicians and other healthcare team members.
• Documents evaluations, progress notes and discharge summaries timely.
• Delivers a full range of speech therapy modalities.
• Assists with clinical affiliations of speech therapy assistants.
• Conducts clinical assessments to identify patient needs and appropriateness of care.
• Documents plans of care, progress notes, phone orders and other clinical records.
• Ensures delivery of quality rehab services.
• Maintains courtesy and respect toward patients and staff.
• Maintains compliance with company policies and code of conduct.
• Works with facility on case management needs and requirements.
• Other duties may be assigned.
