• Develop and implement individualized SLP treatment plans with measurable goals.
• Deliver direct therapy sessions and document progress in EMR promptly.

• Monitor and report measurable outcomes and KPI progress in the territory.
• Develop and implement individualized SLP treatment plans and goals.

• Collaborate with nursing, OT/PT, and care teams to align care plans.
• Utilize clinical pipelines to identify residents in need and prioritize caseload.

• Document treatment plans, progress notes, and discharge summaries in EMR promptly.
• Provide family and caregiver education on communication strategies and progress.

Pay: $2,700-2,800 per week
Contract Length: 13 weeks (extension possible)
Schedule: Day shifts | 36–40 hours per week
