Speech-Language Pathologists (SLP): Travel Contract

Pay: $2,700-2,800 per week

Contract Length: 13 weeks (extension possible)

Schedule: Day shifts | 36–40 hours per week

We’re looking for a motivated and compassionate Speech-Language Pathologists (SLP) to join our Skilled Nursing Facility in San Jose, CA. This 13-week travel assignment offers competitive weekly pay, flexible scheduling, and the opportunity to make a meaningful impact in patient care. Whether you’re an experienced Outpatient therapist or a new graduate with Outpatient clinical rotations, we encourage you to apply.

Responsibilities

Assess and diagnose speech, language, and cognitive disorders in SNF residents.

Monitor and report measurable outcomes and KPI progress in the territory.

Develop and implement individualized SLP treatment plans and goals.

Deliver direct therapy sessions to residents and document progress.

Ensure compliance with regulatory and facility-specific documentation standards.

Collaborate with nursing, OT/PT, and care teams to align care plans.

Utilize clinical pipelines to identify residents in need and prioritize caseload.

Participate in ongoing professional development and stay current with evidence-based practices.

Document treatment plans, progress notes, and discharge summaries in EMR promptly.

Provide family and caregiver education on communication strategies and progress.

Maintain accurate scheduling and on-site availability to maximize patient encounters.

Experience/Qualifications

Licensed Speech-Language Pathologist with SNF experience.

Clinical evaluation and differential diagnosis of speech, language, and cognitive disorders.

Develop and implement individualized SLP treatment plans with measurable goals.

Deliver direct therapy sessions and document progress in EMR promptly.

Comply with regulatory and facility-specific documentation standards.

Collaborate with nursing, OT/PT, and care teams to align care plans.

Engage in ongoing professional development and stay current with evidence-based practices.
