Job summary

Glengariff Healthcare Center, a premier Sub-acute Rehabilitation & Long Term Care facility, seeks a Speech Language Pathologist to assess and treat patients in our skilled nursing setting.

The SLP will develop and administer individualized therapy plans, document progress, and collaborate with an interdisciplinary team to promote patient independence.

NY licensure required; prior long-term care experience preferred; compensation up to $45/hour with an equal opportunity employer.

Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Valid NY license and registration.
• Experience in long-term care and/or geriatric care preferred.
• Strong documentation skills.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer therapy programs with specific goals.
• Document patient condition and educate patients and families.
