Job summary

Glengariff Rehabilitation and Healthcare Center in Glen Head, NY seeks a Speech Language Pathologist (SLP) to assess patients, establish treatment programs, and educate patients and families. The clinician will work with an interdisciplinary team to improve patient function and quality of life in a top-tier sub-acute and long-term care facility.

The SLP should be licensed in NY, graduate from an accredited program, and have strong documentation skills with experience in long-term or geriatric

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time; adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families.
• Collaborate with interdisciplinary team to formulate facility wide programming and processes.
