• Collaborates with all disciplines to provide patient-centered treatment.

Responsibilities

• Assess referred patients requiring rehabilitation and establish treatment goals.
• Develop and administer a patient treatment program under physician direction.
• Document patient condition, progress, and educate patients and families.
• Collaborate with an interdisciplinary team to improve life and function.
