Concept Rehab is seeking a PRN Speech-Language Pathologist (SLP) to join our rehabilitation team at a skilled nursing facility in Johnston, Iowa. This flexible, supplemental role offers schedule control and the opportunity to impact residents' communication, swallowing, and cognitive health.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC-SLP preferred.
• Iowa SLP license or eligibility required.
• Experience in skilled nursing/geriatric settings preferred.
• Strong communication, documentation, and clinical decision-making skills.
• Passion for providing exceptional patient care.

Responsibilities

• Evaluate residents and establish individualized treatment plans.
• Provide speech, language, cognitive, communication, and swallowing therapy services.
• Complete accurate and timely documentation, progress notes, and billing.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care planning and discharge planning meetings.
• Educate residents, caregivers, and facility staff on treatment strategies and maintenance programs.
• Monitor patient progress and modify treatment plans as appropriate.
• Maintain compliance with all federal, state, facility, and company guidelines.
• Support a positive, patient-centered rehabilitation environment.

Position Summary
