Job summary

Concept Rehab, Inc. seeks a PRN Speech-Language Pathologist to join our rehabilitation team at a skilled nursing facility in Johnston, Iowa. This role offers flexible scheduling and the opportunity to deliver high-quality speech therapy services for geriatric residents.

The SLP evaluates and treats residents with communication, cognitive, swallowing and speech disorders while collaborating with the care team to maximize outcomes.

Qualifications

• Master's in Speech-Language Pathology required.
• CCC-SLP preferred but not mandatory.
• Current Iowa SLP license or eligibility to obtain licensure required/preferred.

Responsibilities

• Evaluate residents and establish individualized treatment plans.
• Provide speech, language, cognitive, communication and swallowing therapy.
• Complete accurate and timely documentation, progress notes, and billing.
• Collaborate with physicians, nursing staff, families and interdisciplinary team members.
• Participate in care planning and discharge planning meetings.
• Educate residents, caregivers and facility staff on treatment strategies.
• Monitor patient progress and adjust treatment plans as needed.
