Job summary

Concierge Home Care in Pensacola, FL is seeking a Home Health Speech-Language Pathologist (SLP) to deliver 1:1 patient care in the home with flexible, PRN or 1099 options. Earn $75+ per visit with potential Medicare differentials and mileage reimbursement, while designing your own schedule.

You will assess speech, language, cognition and swallowing, create individualized plans, educate patients and families, and document with AI-assisted tools.

Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations
• Develop individualized treatment plans based on physician orders
• Provide therapy for communication, cognition, and dysphagia
• Educate patients and caregivers to improve functional outcomes
• Collaborate with physicians and interdisciplinary teams
• Complete OASIS documentation as required
• Document visits using streamlined, AI-assisted technology
