Job summary

Concierge Home Care is seeking a Speech-Language Pathologist (SLP) to provide 1:1 home health therapy across Jackson, Calhoun, Liberty, Washington, and Holmes Counties in Florida. This PRN/flexible schedule role offers pay-per-visit compensation with Medicare differential pay and mileage reimbursement.

You will evaluate and treat speech, language, cognitive, and swallowing disorders, develop plans, and educate families while using AI-assisted charting and WellSky/Kinnser EMR to streamline visits.

Qualifications

• Active Florida Speech-Language Pathologist license is required.
• Valid driver’s license and reliable transportation are required.
• Home health experience preferred but not required; OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia in the home setting.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams; complete OASIS documentation as required.
• Document visits using AI-assisted technology.
