Job summary

Concord Post-Acute is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a per diem basis. Location: Concord, CA. Per diem schedule up to 3 days a week, 4-6 hours per day.

Pay starts at $75 per hour. The SLP will evaluate and treat residents with communication, cognitive, voice, language, and swallowing disorders, collaborating with physicians and rehab staff to promote optimal outcomes.

Qualifications

• Current California Speech-Language Pathologist license required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide speech, language, cognitive, communication, and swallowing therapy services.
• Assess and manage dysphagia and recommend dietary modifications.
• Maintain accurate and timely clinical documentation.
• Collaborate with interdisciplinary team members to coordinate resident care.
• Educate residents, families, and caregivers regarding treatment goals and progress.
• Participate in care planning and discharge planning as needed.
• Ensure compliance with regulatory, ethical, and facility standards.

Position Summary

The Speech-Language Pathologist (SLP) evaluates, develops, and implements individualized treatment plans for residents experiencing communication, cognitive, voice, language, and swallowing disorders. The SLP collaborates closely with physicians, nursing staff, rehabilitation professionals, residents, and families to promote optimal outcomes.
