Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist to join the in-house SNF team in Ponca City, OK. The role is offered as Part Time or PRN and focuses on delivering direct therapy and ensuring standards of care.

The SLP will evaluate residents, develop targeted goals, and educate patients, families, and staff to maximize daily functional communication. CMS documentation in NetHealth is required.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required or in process.

Responsibilities

• Provide quality speech therapy services in an in-house SNF setting.
• Evaluate data for treatment planning and implement treatment plans with goals.
• Instruct patients, families, and staff to ensure carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident screening to identify needs and ensure timely care.

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• Ongoing continuing education and professional development to maintain current licensure and certification
