Job summary

Evolve Therapy Services in Lawton, OK is seeking a Speech Language Pathologist to join our in-house Skilled Nursing Facility team in a PRN capacity. The role requires a CCC-SLP, active state licensure, and a commitment to resident care and CMS compliance.

You will deliver therapy, develop treatment plans, educate patients and staff, and ensure accurate NetHealth EMR documentation. Growth opportunities are available within the organization.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC-SLP certification required.
• Active SLP license in state(s) of practice.
• Commitment to ongoing continuing education.

Responsibilities

• Provide quality speech therapy services and refer when appropriate.
• Evaluate and collect data for treatment planning.
• Develop and monitor treatment goals and plans.
• Instruct patient, family, and staff on therapeutic techniques.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident screenings to identify needs.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
