Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Ponca City, OK. This part-time/PRN role focuses on delivering high-quality speech therapy, evaluating needs, and coordinating with the care team to ensure effective patient outcomes.

The position requires a Master’s in Speech Language Pathology, CCC-SLP, and active licensure with ongoing professional development.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC-SLP certification required.
• Active state licensure in practiced states.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and collect data for treatment planning.
• Develop treatment plans with target, short-term and long-term goals.
• Instruct patients, families, and staff for home carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and program coordination.
• Participate in resident needs screening to ensure timely care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Part Time or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
