Evolve Therapy Services in Ponca City, OK seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility. Part-Time or PRN position; you will provide quality therapy services and collaborate with staff to support resident communication needs.

Requirements include a Master's degree in Speech-Language Pathology, CCC, and an active SLP license in the state(s) of practice; ongoing CE/PD maintaining licensure.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required.
• Active SLP license in the state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services in accordance with established standards and productivity expectations.
• Evaluate and collect data necessary for treatment planning and implementation.
• Develop treatment plans with targets, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover into daily activities.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident needs screening to ensure timely delivery of services.

Job Type

Job Purpose

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
