Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Midwest City, OK. This PRN role focuses on planning, developing, and directing speech-language services to ensure high-quality resident care in compliance with CMS and state regulations.

The successful candidate will hold a Master's in SLP, CCC, and active state licensure; will evaluate patients, create treatment plans with measurable goals, and document per NetHealth EMR requirements.

Qualifications

• Master's degree in Speech-Language Pathology required.
• CCC credential (ASHA) required.
• Active state licensure in practicing states required.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate and obtain data for treatment planning and implementation.
• Develop treatment plans with target list, short- and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete CMS-compliant documentation in NetHealth EMR per regulations.
• Oversee FEES/MBS protocols with site program coordination.
• Participate in resident need screening to ensure timely care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
