Job summary

Evolve Therapy Services, LLC is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Ponca City, OK. This position is offered as Part Time or PRN with a focus on high-quality resident care and compliant clinical documentation.

The role requires a Master's degree in Speech-Language Pathology, CCC, and an active SLP license in the state. You will assess residents, develop treatment plans, and collaborate with staff to implement therapy goals within CMS guidelines.

Qualifications

• Master's degree in Speech-Language Pathology.
• CCC required (Certificate of Clinical Competency).
• Active SLP license in state(s) of practice.
• Ongoing CEU/PD to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with targets, short- and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover.
• Complete compliant CMS-directed documentation in NetHealth EMR.
• Oversight of FEES/MBS protocols with site coordination.
• Participate in resident needs screening to ensure timely care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Part Time or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
