Job summary

Evolve Therapy Services in Sutherland, NE is seeking a licensed Speech Language Pathologist to provide in-house therapy at our skilled nursing facility on a PRN basis.

Responsibilities include evaluating residents, developing treatment plans and goals, delivering therapy, documenting in NetHealth EMR, and coordinating with the care team. Requires Master’s in Speech Language Pathology, CCC, and an active SLP license in the state(s) of practice.

Qualifications

• Master’s degree in Speech Language Pathology required.
• CCC and active state licensure are mandatory.
• Continuing education to maintain licensure is expected.

Responsibilities

• Provide quality speech therapy services and initiate referrals as appropriate.
• Evaluate and collect data for treatment planning and progress.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families and staff to ensure carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screening to identify needs and ensure timely care.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
