Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to work in-house at a Skilled Nursing Facility in Midwest City, OK on a PRN basis.

You will plan, organize, and direct SLP services, evaluate residents, develop treatment plans with goals, document to CMS standards in NetHealth EMR, and coordinate FEES/MBS as needed.

Requires a Master's in Speech Language Pathology, CCC, active SLP license, and ongoing CEU participation.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC licensure and state licensure active in practice.
• Commitment to ongoing education and professional development.
• Experience with CMS documentation and EMR systems preferred.

Responsibilities

• Provide quality speech therapy services per standards and productivity.
• Evaluate residents and gather data for treatment planning.
• Develop treatment plans with short- and long-term goals.
• Educate patient, family, and staff to ensure carryover of therapy.
• Complete CMS-mandated documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in resident screenings to identify needs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.

• To evaluate and obtain data necessary for treatment planning and implementation

• To develop a treatment plan that includes a target list, short-term, and long-term goals

• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities

• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems

• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs

• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology

• Certificate of Clinical Competency

• MUST have active SLP license in state(s) of practice.

• Ongoing continuing education and professional development to maintain current licensure and certification
