Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist for an in-house Skilled Nursing Facility in Ponca City, OK. This part-time or PRN role focuses on delivering quality speech therapy, evaluating residents, and guiding care plans aligned with CMS expectations. Licensure must be active in the state of practice.

The position emphasizes collaboration with patients, families, and staff, with documentation in NetHealth EMR and adherence to standard protocols for FEES/MBS and screenings.

Qualifications

• Master's degree in Speech Language Pathology required.
• CCC (Certificate of Clinical Competency) preferred or eligible for licensure.
• Active SLP license in applicable state(s) required.

Responsibilities

• Provide quality speech therapy services in a SNF setting.
• Assess and collect data for treatment planning and progress.
• Develop and implement individualized treatment plans with goals.
• Educate patient, family, and staff to support carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols with site coordination.
• Participate in resident needs screenings with clinical excellence.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Part Time or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
