Evolve Therapy Services in Humboldt, TN is hiring a Speech Language Pathologist for an in-house Skilled Nursing Facility. This full-time role focuses on delivering high-quality therapy, evaluating residents, and coordinating care within CMS guidelines.

You will develop treatment plans with short- and long-term goals, educate patients, families, and staff, and ensure accurate NetHealth EMR documentation. A Master's in Speech-Language Pathology and CCC are required.

Qualifications

• Master's degree in Speech-Language Pathology is required.
• CCC certification is required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide quality speech therapy and initiate referrals per standards.
• Evaluate data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals.
• Instruct patients, families, and staff for carryover of skills.
• Complete CMS-compliant documentation in NetHealth EMR.
• Oversee FEES/MBS protocols and site coordination.
• Participate in screenings to assess resident needs.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility
Job Type: Full Time
Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.
Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
