Evolve Therapy Services in Memphis, TN seeks a Speech Language Pathologist to provide in-house services at a Skilled Nursing Facility. Full-time position with a $3,000 sign-on bonus; PRN option also available.

Responsibilities include evaluating residents, developing treatment plans, and documenting in NetHealth EMR. Must hold a Master’s in Speech Language Pathology, CCC-SLP, and active state licensure, with ongoing professional development.

Qualifications

• Master's degree in Speech-Language Pathology is required.
• CCC-SLP certification is required or in progress.
• Active state licensure in practice state is required.
• Ongoing continuing education to maintain licensure is expected.

Responsibilities

• Provide quality speech therapy services and initiate referrals.
• Evaluate residents and collect data for treatment planning.
• Develop short- and long-term treatment plans with goals.
• Instruct patients, families, and staff on speech-language procedures.
• Complete documentation in NetHealth EMR per CMS expectations.
• Oversee FEES/MBS protocols and coordinate site programs.
• Screen residents to identify needs and ensure timely care.

Job Position: Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type: Full Time ($3,000 sign on!) or PRN

Job Purpose: To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
