Job summary

Evolve Therapy Services is seeking a Speech Language Pathologist to join an in-house Skilled Nursing Facility in Phelps, KY. This part-time or PRN position focuses on delivering quality speech therapy services and coordinating with CMS-compliant documentation.

Requirements include a Master's in Speech Language Pathology, CCC, and an active SLP license in the state(s) of practice. Ongoing CEU/professional development supported by opportunities for growth.

Qualifications

• Master's degree in Speech Language Pathology is required.
• Certificate of Clinical Competency (CCC) is required.
• Active SLP license in the state(s) of practice is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice and department procedures.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop treatment plans with target lists, short-term and long-term goals.
• Instruct patient, family, and staff in appropriate speech-language procedures for daily carryover of therapy.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure resident needs are met timely and with clinical excellence.

Education

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
