Job summary

Evolve Therapy Services, LLC seeks a Speech Language Pathologist for an in-house Skilled Nursing Facility in Andersonville, TN. You'll provide comprehensive therapy, evaluate data, and develop plans to improve resident communication and swallowing outcomes.

You will educate residents, families, and staff, while ensuring CMS documentation in NetHealth EMR and adherence to licensure requirements. Flexible schedule options (Full/Part/PRN) are available with growth opportunities.

Qualifications

• Master’s degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate, per standards and procedures.
• Evaluate and obtain data for treatment planning and implementation.
• Develop a treatment plan with target list, short-term and long-term goals.
• Instruct patient, family, and staff in speech-language procedures for carryover in daily activities.
• Complete CMS-compliant, medically necessary documentation in NetHealth EMR.
• Oversight and implementation of FEES/MBS protocols with site coordination.
• Participate in screening protocols to ensure timely resident needs are met with clinical excellence.

Job Position:

Speech Language Pathologist for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Speech Language Pathology services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.

Job Functions:

• To provide quality speech therapy services and initiate referrals when appropriate, in accordance with established standards of practice, therapy department procedures and productivity standards.
• To evaluate and obtain data necessary for treatment planning and implementation
• To develop a treatment plan that includes a target list, short-term, and long-term goals
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities
• To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
• Oversight and implementation of proper protocols for FEES/MBS with coordination of site programs
• Participants in screening protocols for establishing needs of residents are being met timely and with clinical excellence

Education & Qualifications:

• Master's degree in Speech Language Pathology
• Certificate of Clinical Competency
• MUST have active SLP license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification
