Job description

BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist (SLP) for a per diem opportunity performing home health visits at our office located in Roxboro, North Carolina.

Our office services adult and geriatric patients in homes throughout Caswell, Person, and Orange counties.

Prior home care experience as a Speech Language Pathologist is a plus but not required. As a home care SLP, you will be an integral part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing difficulties.
3. Develop treatment plans based on evaluations, physician's orders, and goals.
4. Assess and revise care plans, collaborating with BAYADA clinicians as appropriate.
5. Educate patients, families, or representatives on rehabilitative care and activities.
6. Document observations, interventions, and evaluations accurately using electronic devices.

Qualifications:

• Current North Carolina license as a Speech Language Pathologist.
• American Speech and Hearing Association certification or equivalent educational and work experience, with eligibility to take the certification exam.
• Solid computer skills; prior EMR experience preferred.
