Job description

BAYADA Home Health Care

We are seeking an experienced Speech Language Pathologist (SLP) for a full-time position providing home health visits at our office in Cranford, NJ. The office serves adult and geriatric patients across Somerset and Middlesex Counties.

Prior home care experience is a plus but not required. As an SLP in home care, you will be a vital part of a multidisciplinary team delivering rehabilitative care and skilled nursing, enabling clients to remain at home.

Responsibilities:

1. Make home visits to clients within designated areas.
2. Conduct evaluations to diagnose and treat speech, language, communication, or swallowing issues.
3. Create individualized treatment plans based on evaluations, physician’s orders, and goals.
4. Regularly assess and update therapy plans, collaborating with other clinicians as needed.
5. Educate patients and their families on rehabilitative activities to promote health and independence.
6. Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current NJ State license as a Speech Language Pathologist.
• Certificate of Clinical Competence from ASHA, or equivalent education and experience, with eligibility to sit for the ASHA exam.
• Strong computer skills; experience with EMR systems is preferred.
