Job description

BAYADA Home Health Care

is currently seeking an experienced Speech Language Pathologist, SLP for a full-time opportunity performing home health visits for our office located in Cranford, NJ.

This office works with adult and geriatric patients in homes throughout Somerset and Middlesex Counties. Prior home care experience as a Speech Language Pathologist, SLP, is a plus but not required. As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the medical care needed to remain at home.

Responsibilities:
  1. Make home visits to clients in designated geographic territories.
  2. Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
  3. Develop treatment plans based on evaluations, physician's orders, and goals.
  4. Assess and revise care plans, participating in multidisciplinary team meetings as appropriate.
  5. Educate patients, family members, or caregivers in rehabilitative care and activities to promote health, safety, and independence.
  6. Document observations, interventions, and evaluations accurately using a state-of-the-art tablet.
Qualifications:
  1. Current NJ license as a Speech Language Pathologist.
  2. Certificate of Clinical Competence in speech from the American Speech-Language-Hearing Association, or equivalent educational and work experience, with eligibility to take the ASHA certification exam.
  3. Strong computer skills; experience with electronic medical records (EMR) preferred.
