**Responsibilities**

Evaluate and treat home health patients requiring speech-language therapy.

+ Perform therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.

+ Educate and counsel patient and family regarding treatment plans.

+ Maintain clinical records and follow performance improvement recommendations.

+ Plan and/or assist with patient discharge from home health speech therapy services.

+ Provide information regarding appropriate support programs.

**Requirements**

Licensure / Certification / Registration:

+ BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

+ Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.

+ Speech-Language Pathologist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.

Education:

+ Bachelor's degree required.

+ Master's degree preferred.

**Additional Preferences**

+ Bachelors in Speech pathology Required.

+ Minimum of 2 years of geriatrics experience strong preferred.

+ Home Health experience Strongly Preferred.

+ Daytime Weekdays- 6 visits a week, no weekends or holidays, flexible schedule.

+ Pay Rate times Visit multiplier.
