LifeSpan Therapy in Fishers, Indiana is seeking a Speech Language Pathologist to assess, diagnose, and treat residents with speech, language, and swallowing disorders, collaborating with a multidisciplinary team to tailor treatment plans. You will support residents’ communication and independence through evidence-based therapy.

Qualifications

  • Must be a licensed therapist who is a graduate of an accredited school.
  • Must have a valid Indiana Operator's license and be able to travel occasionally to other locations if needed to provide therapy coverage.
  • Basic computer literacy for documentation and communication purposes.
  • Experience with outpatient or rehabilitation therapy is preferred.
  • Familiarity with EHR systems is preferred.
  • Strong interpersonal skills with experience in resident and family education.

Responsibilities

  • Conduct comprehensive evaluations of residents' speech, language, cognitive-communication, and swallowing abilities.
  • Coordinate admission/discharge to speech therapy with the Interdisciplinary Team and nursing staff.
  • Provide therapy to residents under medical orders in collaboration with the Interdisciplinary Care Plan Team.
  • Monitor resident progress and adjust therapy techniques to achieve optimal outcomes.
  • Educate residents, families, and caregivers on communication strategies and exercises.
  • Maintain accurate documentation of evaluations, treatment plans, and progress notes in compliance with regulatory standards.
  • Collaborate with physicians, occupational therapists, and physical therapists to coordinate resident care.

About the Role

As a Speech Language Pathologist at LifeSpan Therapy, you will play a critical role in assessing, diagnosing, and treating individuals with speech, language, communication, and swallowing disorders. Your work will directly impact the quality of life for res i d ents by helping them improve their communication abilities and overall functional independence. You will collaborate closely with a multidisciplinary team to develop personalized treatment plans tailored to each res i d ent's unique needs. This role requires a compassionate approach to res i d ent care, ensuring that therapy sessions are both effective and supportive. Ultimately, your expertise will contribute to the advancement of res i d ent outcomes and the enhancement of their social and emotional well-being.

Job Responsibilities

  • Conduct comprehensive evaluations of res i d ents' speech, language, cognitive-communication , and swallowing abilities.
  • Coordinates admission/ discharge to speech therapy with the Interdisciplinary Team, Restorative Care Nurse and staff nurses.
  • Provides therapy to residents under specific medical orders and in collaboration with the Interdisciplinary Care Plan Team.
  • Monitor res i d ent progress and adjust therapy techniques as necessary to achieve optimal outcomes.
  • Educate res i d ents, families, and caregivers on communication strategies and therapeutic exercises to support ongoing improvement.
  • Maintain accurate and timely documentation of res i d ent evaluations, treatment plans, and progress notes in compliance with regulatory standards.
  • Collaborate with healthcare professionals, including physicians, occupational therapists, and physical therapists, to coordinate res i d ent care.

Minimum Qualifications

  • Must be a licensed therapist who is a graduate of an accredited school.
  • Must have a valid Indiana Operator's license and be able to travel occasionally to other locations if needed to provide therapy coverage.

Preferred Qualifications
