Centra Healthcare is seeking an experienced Speech Language Pathologist (SLP) for a contract assignment in a SNF/LTC setting. The role emphasizes accountability and aligning with the organization’s vision and goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Speech Language Pathologist / SLP or in process for licensure.
• Current CPR certification.
• Work within an interdisciplinary team to develop a plan of care for each patient.
• Provide skilled speech therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of speech therapy.
• Candidate must be an SLP-CCC.

Responsibilities

• Collaborate with the care team to implement and adjust individualized treatment plans.
• Deliver skilled SLP services in accordance with physician orders.
• Set realistic goals to develop or restore communication and swallowing skills.
• Monitor patient progress and update care plans as needed.

A great SNF / LTC is looking for an experienced Speech Language Pathologist / SLP to join their growing team for a contract assignment! This is a contract position. Days schedule. This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include: Must have graduated from an accredited school. Current state license as a Speech Language Pathologist / SLP or proactively in the process of the application process for current state licensure as a Speech Language Pathologist / SLP. Current CPR certification. Work within an interdisciplinary team to develop a plan of care for each patient. Provide skilled speech therapy services / interventions in accordance with physician orders. Assesses patient needs, plans for, evaluates and modifies care to meet goals of speech therapy interventions. Set realistic and achievable goals for patients to develop, or recover, reliable communication and swallowing skills so that the patient can fulfill their educational, vocational and social roles. Candidate must be an SLP-CCC.
