Job summary

Richmond Post Acute in Richmond, VA is seeking a full-time Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team. You will evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, voice, and swallowing disorders, and educate families about home exercises.

Virginia licensure is required; CCC-SLP is preferred and CFY candidates are encouraged.

Qualifications

  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current Virginia SLP license.
  • CCC-SLP preferred; CFY candidates encouraged where permitted.
  • Current CPR certification (or willingness to obtain on hire).
  • Experience in skilled nursing or post-acute care preferred.
  • Strong clinical assessment and treatment skills.
  • Excellent communication, organizational, and interpersonal skills.

Responsibilities

  • Develop individualized treatment plans based on physician orders, clinical findings, and resident goals.
  • Provide evidence-based speech-language therapy services per regulations.
  • Collaborate with interdisciplinary team to coordinate resident care.
  • Educate residents, families, and caregivers on treatment plans and home exercise programs.
  • Maintain accurate documentation of evaluations, treatments, progress notes, and discharge summaries.
  • Participate in care plan meetings, discharge planning, and quality improvement initiatives.
  • Ensure compliance with Medicare, Medicaid, HIPAA and regulatory standards.
  • Stay current on best practices through continuing education.

Job Summary:

Richmond Post Acute is seeking a Full Time and PRN (Per Diem) Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team at Richmond Post Acute.

As a key member of our team, you will evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, voice, and swallowing disorders.

Your expertise will play a vital role in helping residents regain independence, improve communication, and enhance their overall quality of life.

Essential Responsibilities:
  • Develop individualized treatment plans based on physician orders, clinical findings, and resident goals
  • Provide evidence-based speech-language therapy services in accordance with state and federal regulations
  • Collaborate with physicians, nursing staff, occupational therapists, physical therapists, dietitians, and other interdisciplinary team members to coordinate resident care
  • Educate residents, families, and caregivers regarding treatment plans, progress, and home exercise programs
  • Maintain accurate, timely, and compliant documentation of evaluations, treatments, progress notes, and discharge summaries
  • Participate in care plan meetings, discharge planning, quality improvement initiatives, and facility education programs
  • Ensure compliance with Medicare, Medicaid, HIPAA, and all applicable regulatory standards
  • Stay current on best practices through continuing education and professional development
Qualifications:
  • Master's Degree in Speech-Language Pathology from an accredited program
  • Current Speech-Language Pathologist license in Virginia
  • Certificate of Clinical Competence (CCC-SLP) preferred; Clinical Fellowship (CFY) candidates are encouraged to apply where permitted
  • Current CPR certification (or willingness to obtain upon hire)
  • Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred
  • Strong clinical assessment and treatment skills
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work independently while collaborating effectively within an interdisciplinary team
