Job summary

Highland Springs Post Acute in Highland Springs, VA is seeking a compassionate Speech-Language Pathologist (SLP) to evaluate and treat residents with speech, language, cognitive-communication, voice, and swallowing disorders. You will develop individualized plans and work within an interdisciplinary team to restore communication and independence.

Responsibilities include conducting assessments, implementing evidence-based therapy, documenting progress, and ensuring regulatory compliance.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current SLP licensure in the state of employment.
• CCC-SLP preferred; CFY candidates encouraged where permitted.
• Current CPR certification (or willingness to obtain on hire).
• Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred.
• Strong clinical assessment and treatment skills.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently while collaborating within an interdisciplinary team.

Responsibilities

• Evaluate residents to identify speech, language, cognitive, voice, and swallowing impairments.
• Develop individualized treatment plans based on physician orders, clinical findings, and resident goals.
• Provide evidence-based speech-language therapy services in accordance with state and federal regulations.
• Assess and treat dysphagia, recommending appropriate diet modifications and safe swallowing strategies.
• Collaborate with physicians, nursing staff, occupational therapists, physical therapists, dietitians, and other team members to coordinate resident care.
• Educate residents, families, and caregivers regarding treatment plans, progress, and home exercise programs.
• Maintain accurate, timely, and compliant documentation of evaluations, treatments, progress notes, and discharge summaries.
• Participate in care plan meetings, discharge planning, quality improvement initiatives, and facility education programs.
• Ensure compliance with Medicare, Medicaid, HIPAA, and all applicable regulatory standards.
• Maintain a safe, clean, and organized treatment environment.
• Stay current on best practices through continuing education and professional development.
