Job summary

Eden Senior Care in Minnesota is seeking a PRN Speech-Language Pathologist to provide therapy in our skilled nursing facilities. You will perform interventions, document patient progress, and collaborate with the care team to support recovery and daily communication goals.

A Master’s degree in Speech-Language Pathology and active Minnesota SLP licensure are required; experience in long-term care is preferred.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active Minnesota SLP license.
• Experience in long-term care or skilled nursing preferred.

Responsibilities

• Performs therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.
• Communicates patient progress to supervisor and team; assists with scheduling and post charges to patient records.
• Instructs patient’s family or nursing staff in follow-through programs.
• Evaluates and gathers data for treatment planning and implementation.
• Develops a treatment plan with target list, short- and long-term goals.

Primary Job Responsibilities:

• Performs therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.
• Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records.
• Instruct patient's family or nursing staff in follow-through programs.
• To evaluate and obtain data necessary for treatment planning and implantation.
• To develop a treatment plan that includes a target list, short-term and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in dally activities.

Qualifications

• Master's degree in Speech-Language Pathology (SLP) from an accredited program.
• Active Minnesota Speech-Language Pathologist (SLP) license.
• Experience in long-term care or a skilled nursing facility is preferred.
