Job summary

Eden Senior Care, LLC in Minnesota is seeking a PRN Speech-Language Pathologist to provide therapy interventions and contribute to patient care in skilled nursing and related settings.

You will collaborate with the supervisor and care team, document progress, and assist with scheduling and family education as part of a flexible PRN schedule.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Active Minnesota Speech-Language Pathologist (SLP) license.
• Experience in long-term care or skilled nursing facility preferred.

Responsibilities

• Performs therapy interventions utilizing standard therapy techniques and skills as appropriate for the condition of the patient.
• Communicate patient progress or problems to supervisor and other team members; assist with patient scheduling and post charges daily to patient records.
• Instruct patient's family or nursing staff in follow-through programs.
• To evaluate and obtain data necessary for treatment planning and implantation.
• To develop a treatment plan that includes a target list, short-term and long-term goals.
• Instructs patient, family, and staff in appropriate speech-language procedures to allow carry over of therapeutic remediation and goals in daily activities.
