Job summary

Desert Peak Care Center in Phoenix, AZ is seeking a Speech Language Pathologist (SLP) to evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. You will work with an interdisciplinary team to help residents regain independence and improve quality of life.

The role involves developing evidence-based treatment plans, educating residents and families, and ensuring documentation compliance.

Qualifications

  • Master’s degree in Speech-Language Pathology is required.
  • Current Arizona licensure as a Speech-Language Pathologist (or eligibility) is required.
  • CCC-SLP certification is preferred.
  • CPR certification is required or willingness to obtain.
  • Experience in a skilled nursing facility or with geriatric populations is preferred.

Responsibilities

  • Evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders.
  • Provide direct therapy services in accordance with the plan of care.
  • Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive resident care.
  • Educate residents, families, and staff on communication strategies and safe swallowing techniques.
  • Document all evaluations, treatments, and resident progress in compliance with state, federal, and facility regulations.
  • Participate in care planning meetings, discharge planning, and quality improvement activities as needed.
  • Provide training and supervision to Speech-Language Pathology Assistants (if applicable).

Education

Master’s degree in Speech-Language Pathology
Arizona licensure eligibility (or current licensure)
CCC-SLP certification (preferred)
CPR certification (current or willingness to obtain)
Experience in skilled nursing facility or geriatric populations (preferred)

Position Summary:

As an SLP, you will evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders. You will work closely with an interdisciplinary team to help residents regain their independence and improve their quality of life.

Key Responsibilities:
  • Conduct evaluations and develop individualized treatment plans for residents with speech, language, cognitive, and swallowing disorders.
  • Provide direct therapy services in accordance with the plan of care.
  • Collaborate with physicians, nurses, and other rehabilitation professionals to ensure comprehensive resident care.
  • Educate residents, families, and staff on communication strategies and safe swallowing techniques.
  • Document all evaluations, treatments, and resident progress in compliance with state, federal, and facility regulations.

  • Current Arizonalicensure as a Speech-Language Pathology (or eligibility to obtain).
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred;
  • Current CPR certification (or willingness to obtain).
  • Experience in a skilled nursing facility or with geriatric populations preferred.
