Job summary

21st Century Home Health Services (21HHS) is seeking a Speech Therapist to provide in-home services across the Bay Area, especially Sacramento County: Oakland. You will evaluate and treat speech, language, cognitive, and swallowing disorders with the Case Manager, serving as an educator and patient advocate within an interdisciplinary team.

This role offers flexible scheduling, mileage reimbursement, and comprehensive benefits including 401k with matching. CA licensure and BLS/CPR are required.

What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team

To be a success as Speech Therapist/Language Pathologist (ST)

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
