Job summary

Serenity-Therapy-Solutions-LL in Queens,NY is seeking a full-time Speech-Language Pathologist to provide comprehensive SLP services in a skilled nursing facility. The role includes patient evaluation, data collection, referrals, and ongoing documentation to support the plan of care.

The ideal candidate will hold NYS licensure with CCC-SLP and have experience in cognitive and dysphagia rehab within SNFs, with familiarity of Rehab Optima preferred.

Qualifications

• Valid NYS License with CCC-SLP certification.
• Experience in short-term and long-term SLP care in SNF settings.
• Preferred knowledge of Rehab Optima software.

Responsibilities

• Provide quality SLP services, assess patients, initiate referrals, collect data, and follow through with plan of care.
• Complete timely documentation: progress notes, screen reports, daily notes, discharge summaries.
• Manage cognitive and dysphagia rehabilitation within SNF.
• Instruct patients, families, and staff on goals, progress, and techniques.
• Ensure compliance with protocols and regulations.
