Job summary

Flagstar Rehab is hiring a Speech Language Pathologist (SLP) for a Skilled Nursing Facility in the NYC area. You will perform evaluations, develop plans of care, and provide therapy to SNF patients with conditions such as stroke, TBI, Parkinson’s, and MS, using EMR systems.

We offer full‑time positions with flexible schedules and competitive hourly pay. Direct employment or independent contractor options are available, with weekly pay and supportive team environment.

Qualifications

• Current NYS SLP License and Registration.
• Clinical Competence Certification (CCC).
• CPR Certificate.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.

Responsibilities

• Provide efficient speech or swallow evaluations and/or screens of patients.
• Develop plan of care, downgrade/upgrade as appropriate, manage meal texture.
• Provide quality therapy services to individuals in an SNF setting, covering conditions such as TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of EMR software.

Speech Language Pathologist (SLP) – Skilled Nursing Facility

Now Staffing Full Time Speech Language Pathologists for Rehab Department in state‑of‑the‑art SNFs throughout NYC and surrounding areas. Flagstar Rehab SLPs work with a diverse and multi‑disciplinary team to perform evaluations, treatments, and patient/family education to improve speech and swallow capabilities and functional daily life. Not accepting Clinical Fellows (CFY) at this time.

Employment Options

Compensation

Hourly Pay Rate: $50 Hourly (W‑2), $65 Hourly (1099)
