Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities across Long Island, NY, including Mastic Beach and Far Rockaway. Join a compassionate team delivering one-on-one therapy in patients’ homes to improve communication, speech, cognition, and swallowing skills.

Responsibilities include patient evaluation, treatment planning, delivering therapy, progress monitoring, and thorough documentation.

Qualifications

• Active New York State Speech-Language Pathologist License required.

Homeside Rehab is seeking a licensed Speech-Language Pathologist (SLP) for home care opportunities in Long Island, NY.

We are looking for compassionate and dependable clinicians who are passionate about providing high-quality, one-on-one care while helping patients improve communication, speech, language, cognition, and swallowing skills in the comfort of their homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans

• Provide skilled speech-language therapy based on each patient’s needs and goals

• Monitor patient progress and adjust treatment plans as appropriate

• Complete accurate and timely documentation

• Communicate effectively with office staff, patients, and caregivers

• Collaborate with the clinical team to ensure coordinated and effective treatment
