Job summary

LiveWell Homecare Agency, LLC is seeking a Home Health Speech Language Pathologist (SLP) to provide personalized therapy in patients’ homes across Colorado, focusing on communication, swallowing, and cognitive impairments after illness or injury.

You will assess, plan, and document progress, deliver exercises for speech, swallowing, and cognition, educate families on care plans, and collaborate with a multidisciplinary team to ensure patient-centered care.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology and Colorado license.
• At least one year of experience in speech therapy, preferably in home healthcare.
• CPR certification and ability to pass background and drug tests.
• Knowledge of home health regulations, strong clinical judgment, empathy, and excellent communication skills.
