Job summary
Starlight Home Health is seeking a compassionate Speech-Language Pathologist (SLP) to provide therapy services to patients in their homes. This role supports patients’ safety, communication, and quality of life with independent work in a home health setting.
The ideal candidate holds a Colorado SLP license, a graduate degree in Speech-Language Pathology, CPR/BLS, and a willingness to travel to patients’ homes. Flexible scheduling and a supportive team are offered.
Qualifications
- Current Colorado Speech-Language Pathologist license.
- Graduate degree in Speech-Language Pathology from an accredited program.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and willingness to travel to patients' homes.
- Strong communication and documentation skills.
- Home health experience preferred, but not required.
Responsibilities
- Complete speech therapy evaluations and reassessments.
- Develop individualized plans of care and treatment goals.
- Provide treatment for speech, language, cognitive-communication, and swallowing disorders.
- Provide dysphagia assessment, treatment, and education as appropriate.
- Educate patients, families, and caregivers.
- Communicate changes in condition and recommendations to providers and the interdisciplinary team.
- Complete accurate and timely home health documentation.
- Coordinate care with nursing, PT, OT, providers, and other members of the care team.
