Countryside Post-Acute and Rehabilitation Center in Tennessee seeks a Speech Language Pathologist (SLP) to evaluate and diagnose communication and swallowing disorders, develop individualized treatment plans, and provide direct therapy to diverse patient populations. The role requires a Master’s in SLP, CF completion, and current Tennessee licensure, with the ability to work in a fast-paced
Evaluate and diagnose communication and swallowing disorders in patients with a range of conditions and diagnoses
Develop and implement individualized treatment plans to address patient communication and swallowing needs
Provide speech therapy services to patients, including individual and group sessions, as well as carry-over exercises and activities
Collaborate with other healthcare professionals to ensure comprehensive care and goal achievement
Document patient progress and treatment plans in accordance with facility policies and procedures
Participate in patient care conferences and family meetings to discuss treatment progress and goals

Master's degree in Speech-Language Pathology (SLP)
Clinical fellowship (CF) complete
Current licensure or certification in the state of Tennessee
