Job summary

West Tennessee Post-Acute in Jackson, TN, is seeking a Speech Therapist to diagnose and treat patients with communication and swallowing disorders. You will work with an interdisciplinary team to create tailored treatment plans and deliver evidence-based therapy in a patient-centered environment.

The role requires a Master's in speech pathology with state licensure and strong communication skills. Experience in SNF/long-term care is a plus.

It has been CMS-certified since 1986 and offers transitional care services aimed at helping patients recover after hospital stays. Position Overview:

As a Speech Therapist (ST) , you will play an integral role in diagnosing and treating individuals with communication and swallowing disorders, working collaboratively with interdisciplinary teams to develop comprehensive treatment plans tailored to meet the unique needs of each patient.

Conduct thorough assessments and evaluations of patients with speech, language, and swallowing disorders.

Develop individualized treatment plans based on assessment results and patient goals.

Implement evidence-based therapeutic interventions to address identified communication difficulties.

Monitor and document patients' progress throughout the course of treatment.

Collaborate with physicians, occupational therapists, and other healthcare professionals to provide holistic care.

Educate patients and their families on speech and language disorders and coping strategies.

Maintain accurate and timely documentation of patients' evaluations, treatment plans, and progress notes.

Qualifications:

Master's degree in speech pathology is REQUIRED

State licensure and relevant certifications.

Strong interpersonal and problem-solving skills.

Skilled Nursing or Long-Term Care, experience is a plus
