Speech Language Pathologist (SLP)

We are seeking a dedicated and compassionate Speech Language Pathologist (SLP) to join our interdisciplinary team at Countryside Post‑Acute and Rehabilitation Center in Lawrenceburg, Tennessee. As an SLP, you will play a vital role in helping our patients regain their communication skills and achieve their rehabilitation goals.

Responsibilities

• Evaluate and diagnose communication and swallowing disorders in patients with a range of conditions and diagnoses.
• Develop and implement individualized treatment plans to address patient communication and swallowing needs.
• Provide speech therapy services to patients, including individual and group sessions, as well as carry‑over exercises and activities.
• Collaborate with other healthcare professionals to ensure comprehensive care and goal achievement.
• Document patient progress and treatment plans in accordance with facility policies and procedures.
• Participate in patient care conferences and family meetings to discuss treatment progress and goals.
