Job summary

HSHS Rehabilitation Hospital in Green Bay is seeking a Speech-Language Pathologist for a full-time role. You will evaluate, treat, and document therapy, aligning with physician orders and discharge planning to maximize patient outcomes.

Qualifications

  • Requires a Bachelor's and Master's in Speech-Language Pathology or related field.
  • Possess or eligible for ASHA CCC or CFY eligibility.
  • Licensed or eligible for licensure in state.
  • Current BLS/CPR card.

Responsibilities

  • Conduct speech-language therapy evaluations, modalities and treatments per policy.
  • Perform patient assessment and establish an individualized plan of care.
  • Deliver care according to physician orders and the plan of care.
  • Assess treatment effectiveness and modify as needed to meet goals.
  • Plan for discharge and communicate with patient and care teams to optimize outcomes.

Facility: Hospital Sisters Health System Rehabilitation Hospital, a brand-new 40 bed inpatient rehabilitation facility (IRF) now open! Schedule: Full Time

How you'll Contribute
  • Assumes responsibility and accountability for a designated group of patients and provides speech and language therapy evaluations, modalities, and treatments in accordance with established hospital and departmental policies and procedures, maintaining the highest degree of quality patient care.
  • Performs patient assessment.
  • Establish a plan of care.
  • Provides care in accordance with physician's orders and established plan of care.
  • Assesses the effectiveness of treatment and modifies treatment to achieve goals.
  • Plans for patient discharge. Communicates with patients, family, caregivers, significant other, and members of the healthcare team to achieve rehabilitation goals and promote maximum benefits of care.

What we're looking for
  • A Bachelor's and Master's degree in Speech-Language Pathology, Communication Disorders, Communicative Disorders, or similarly titled area that is consistent and acceptable to the American Speech-Language-Hearing Association.
  • Possess or eligible for a Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or be eligible to participate as an ASHA Clinical Fellow (CFY).
  • Licensed or eligible for licensure in state.
  • Current Basic Life Support/CPR card.
