Job summary

Adelphi Nursing and Rehabilitation Center is seeking a part-time Speech & Language Pathologist (SLP) to join our long-term care team. The role focuses on direct patient care, treatment planning, and collaboration with a multidisciplinary staff to promote patient-centered outcomes.

The ideal candidate will hold an active SLP license in the state and have experience in geriatric care, with a commitment to professional growth within a supportive facility culture.

Qualifications

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred

Responsibilities

• Providing direct patient care to assess medical conditions, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals per patient capacity under physician direction.
• Document and educate patients and families on therapy methods and progress.
• Collaborates with all disciplines to provide patient-centered care.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
