Job summary

Eastern Health Group in Virginia is seeking a PRN Speech Language Pathologist (SLP) to join our resident-centered, quality-focused rehabilitation team. You will assess referred patients, establish and monitor treatment plans, document progress, educate families, and collaborate with all disciplines to provide patient-centered care in a positive work environment.

Qualifications

  • Must have graduated from an accredited Speech Therapist program.
  • Must hold valid licensure/registration as an SLP.
  • Experience in long-term care or geriatrics preferred.

Responsibilities

  • Assessment of referred patients requiring rehabilitation services.
  • Provide direct patient care to assess medical condition, function, limits and rehab potential.
  • Establish and administer a treatment program with goals per patient capacity.
  • Set realistic goals for patients and document progress weekly.
  • Educate patients and families in therapy methods.
  • Complete evaluations, progress notes, recertifications and discharge docs.
  • Collaborate with all disciplines for patient-centered treatment and care.
  • Perform duties assigned by Director of Rehabilitation.
  • Comply with confidentiality, HIPAA and Residents' Rights.

Duties and Responsibilities of the Speech Language Pathologist:

  • Assessment of referred patients that require rehabilitation services
  • Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
  • Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
  • Set realistic and achievable goals for their patients
  • Document and record the patients’ condition
  • Educate patients and families in an appropriate therapy method.
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements of the Speech Language Pathologist:

  • Must have graduated from an accredited Speech Therapist college or university
  • Must have a valid professional license and registration
  • Experience with long-term care and/or geriatric care is preferred but not required
