Job summary

Eastern Health Group is seeking a Speech Language Pathologist (SLP) to assess and treat patients in our rehabilitation-focused facilities in Roanoke, VA. You will work under the direction of physicians to develop and implement individualized therapy plans.

The role emphasizes collaboration with a multidisciplinary team, patient-family education, and thorough documentation of progress and discharge planning.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to evaluate medical condition and rehab potential.
• Establish and administer treatment program with goals per patient capacity.
• Set realistic and achievable goals for patients.
• Document and record patient conditions.
• Educate patients and families in therapy methods.
• Perform evaluations, progress notes, recertification and discharge docs.
• Collaborate with all disciplines for patient-centered care.
• Perform other duties assigned by Director of Rehabilitation.
• Ensure confidentiality and HIPAA compliance.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
