Job summary

Post Rehab is seeking a Speech-Language Pathologist (SLP) to join the rehabilitation team at Epsom Health Care Center. This full‑time opportunity offers the chance to work closely with residents on communication, cognitive, and swallowing needs while being part of a supportive skilled nursing and post‑acute care team.

The ideal candidate has an active NH SLP license and a graduate degree in Speech-Language Pathology, with strong clinical and communication skills.

Qualifications

  • Active New Hampshire Speech-Language Pathologist license required.
  • Graduate degree in Speech-Language Pathology from an accredited program.
  • Strong clinical, communication, and organizational skills.
  • New graduates are welcome to apply.
  • Rehabilitation experience is a plus.

Responsibilities

  • Evaluate residents with speech, language, cognitive-communication, and swallowing disorders.
  • Develop individualized treatment plans based on clinical findings and resident goals.
  • Provide skilled interventions focused on communication, cognition, and dysphagia.
  • Monitor resident progress and adjust treatment approaches as needed.
  • Educate residents, families, and caregivers on therapeutic strategies and recommendations.
  • Complete accurate and timely clinical documentation.
  • Collaborate with PT, OT, nursing, physicians, and other members of the interdisciplinary care team.
